Description

Abstract
食源性有害物包括有機 (如鄰苯二甲酸酯類、雙酚A、三聚氰胺等)及無機污染物(關鍵放射核種 如鑭系、銫、鉀、碘等及關鍵指標元素如鎘、磷、鋁、鉻、錳、銅等) 等,在世界各國多屬優先長期 監測之重要有害物。本研究目的為建立台灣一般國人體內食源性有害物 (如鄰苯二甲酸酯類、雙酚A、 三聚氰胺、關鍵放射核種及指標元素等)於尿液中之背景暴露濃度等,且探討其是否為影響腎功能之危 險因子,及是否受到不同年齡、性別及地區等而影響,藉以進行食源性有害物之每日暴露劑量推估或 參考值建置及預防醫學之建議。 本研提三年計畫將使用國衛院環醫所自104-105 年已建立全國20 縣市約1617 位7 歲以上台灣一般國 人檢體,皆已提供尿液檢體、暴露問卷及同意書檢測上述有害物。將逐年分別以易感(7~<18 歲)、 青壯(18~<50 歲)及中高齡族群(50 歲)為對象,每年約進行400 人且排除腎臟疾病者。尿液樣 本將分別使用液相層析串聯式質譜儀、感應耦合電漿質譜儀/原子發射光譜分析儀及加馬能譜分析儀進 行檢測分析,及檢測尿液中腎功能相關指標如微尿蛋白等。將使用相關暴露或健康風險評估模式,建 立台灣一般國人此四類食源性有害物之人體暴露參考值或每日暴露劑量,並可據以提出適合台灣之建 議每日容許量或參考值等。 Food-originated hazardous pollutants (FoHPs) including organic (such as phthalates, bisphenol A and melamine) and inorganic pollutants (key radiation and indicator elements, such as La, K, Cd, Al, Mn) are on the priority list of human bio-monitoring surveillance in many countries due to their potential health effects in human. The objectives of this study are to determine the exposure levels of above FoHPs, such as phthalates, bisphenol A, melamine, key radiation and indicator elements, in the general Taiwanese population, to establish reference value or estimate daily intake using each levels of urinary metabolites of different FoHPs and existed risk assessment model, and to assess the correlation between levels of FoHPs and indicator of renal function. Besides, we’ll assess the influence of age, gender, region and other factors on exposure levels of FoHPs in the general Taiwanese. We proposed a three-years project to used our established cohort from 2015 to 2016, which included 1617 Taiwanese (7 year old, y) from twenty counties in Taiwan. All participants have provided an informed consent, first-morning urine sample and fill an exposure questionnaire related above FoHPs. We’ll categorize our participant as susceptible population (7~ <18 y), reproductive-aged population (18~ <50 y), and middle-aged/ elder population (50 y). We’ll include around 400 participants, excluded subject with renal diseases, every year from 2017 to 2020. We’ll analyze participant's urine sample to obtain their exposure levels of above FoHPs using LC-MS/MS, ICP/MS, ICP-AES and gamma-ray spectrometer, respectively. We’ll analyze indicators of renal function such as microalbumin and creatinine, in urine sample. We’ll use urinary levels of FoHPs to estimate the reference values or daily intake in different age groups of the general Taiwanese using existed risk assessment models. We’ll suggest reference value or acceptable/tolerable daily dose of FoHPs in different age groups of the general Taiwanese. Our proposed human biomonitoring surveillance of FoHPs in the general Taiwanese will be helpful for evaluating the potential health effects of FoHPs, early warning of potential food contamination and related risk managements, and health promotion in the general Taiwanese.
